A Clinical Study on Primary Carcinoma of the Gallbladder



Abstract
Despite improvements in diagnostic and therapeutic and therapeutic facilities, gallbladder carcinoma patients continue to have an extremely poor prognosis. This fact has not changed since the disease entity was first described more than 200 years
ago,
and is due to late presentation of the disease due to non specific initial complaints.
A review of 34 cases of primary carcinomas of the gallbladder, proven histologically and treated surgically at the Department of surgery, Kyung Sang National University Hospital during a period of 9 years from Jan, 1987 through Dec, 1995, was
undertaken.
@ES The conclusions were as follows;
@EN Sex ratio of male to female was 1 to 1.4 with female predominance. The age with high prevalence was 7th decade. Duration of illness revealed 29cases(85.3%) within the 6 months. The most common clinical manifestation was right upper abdominal
pain in
24 cases(70.6%), and the most common finding of physical examinations was right upper abdominal tenderness in 26 cases (76.5%). The preoperative diagnosis was possible in 79.4% of cases. Diagnostic accuracies of abdominal USG and CT were 75.0%,
85.3%,
respectively. Curative resection was possible in 18 cases(52.9%) and most common metastatic site was liver in 17 cases(50%). Among the coexisting disorders, gall stones were 6 cases(17.7%0, and clonorchiasis were 5 cases(14.7%). Most common
histologic
type (88.2%) was adenocancinoma. Cumulative 2 YSRs was 19.1% obtained by use of the Kaplan Meier method. 2 YSRs of early and advanced cases were 55.6%, 5%, respectively(P<0.05) 2 YSRs of resectable and unresectable cases were 36.4%, and 0%,
respectively(p<0.05). Statistical comparison of the cumulative survival rates was analyzed by use of the log ranksum test. A high index of suspicion of the disease, intraoperative examination of gallbladder specimen, and earlier, and more
aggressive
surgical treatment may improve the patient's survival.
